Scope

Record the staged-projection decisions from the 2026-10-02 architecture grilling:

• docs/adr/0012-staged-projection-and-evidence-ladder.md, amending ADR 0002 and ADR 0003.
• CONTEXT.md terms: Projection stage, Provisional projection, Evidence ladder, Activity readout. Scenario range is updated for short horizons.

Acceptance criteria

• ADR 0012 accepted and linked from its amended ADRs' wording where needed.
• Glossary entries contain no implementation detail.
