fix(docs): rename VocaServer to VocaGateway; reuse GITHUB_REPO_URL (#697)

Browse Source 
Point README/CONTRIBUTING sibling links at VocaHQ/vocagateway, and drive
homepage GitHub links and install/uninstall commands from GITHUB_REPO_URL.
